Diabetic retinopathy and long-term therapy with Doxium 52 patients treated over periods ranging from 5--9 years

The aetiopathogenesis of diabetes mellitus is still obscure numerous factors of various nature have been suggested as possibly having a causative role. Among the factors that may influence the course of diabetes and its vascular complications are: heredity, ethnic, immunologic, psychologic, social, dietetic, traumatic, insulin, drug interaction and others which cannot be discussed here. Our personal experience with calcium dobesilate goes up to 1967. The conjunctival capillary resistance (CCR) which is often markedly diminished in diabetics, is measured routinely in all our studies because it is regarded as a reliable test to visualize the state of the blood vessels. We noticed in our first trials that the conjunctival capillary resistance rapidly returned to normal under the influence of calcium dobesilate, an observation that has since then been confirmed by other investigators. We now have nine years' clinical experience with calcium dobesilate in DR. Our records at the University Eye Hospital, Lausanne comprise 210 patients treated with it, end the results obtained have been published at the various stages of the experimentation. Its angioprotective action in DR has since been confirmed by other investigators in open trials. Of five doubleblind trials reported one only gave a negative result. The dosage used at the beginning of our experience with calcium dobesilate, 500 mg. per day, sometimes proved insufficient and subsequent dosage was determined by the severity of the clinical manifestations. It should be noted that calcium dobesilate does not interfere with the action of insulin and oral hypoglycaemic drugs. Moreover, it does not antagonize the action of the coumarin anticoagulants. Its hepatic innocuousness already stated by Schwaar after one-year treatment has been confirmed in this long-term study with calcium dobesilate on 52 patients by hepatic transminase determinations.
