Current treatment of the pulmonary aspiration of gastric contents

Literature about pathophisiology, clinic and prevention of the pulmonary aspiration of gastric content syndrome, is copious, but in that concerning to the treatment is scant and highly controversial. In order to clear this aspect, we review the current treatment of that syndrome. The simplest and most efficient therapy is prevention. If aspiration does occur, the immediate treatment is to maintain a free airway with intubation, endotracheal suction and bronchoscopy if alimentary particles are suspected to be aspirated. Generally, pulmonary lavages and corticosteroids aren't recommended. Ventilatory support with CPAP, and mechanical ventilation with IMV and PEEP have permited the greatest progress in the treatment of critical forms. Adequate volume replacement with close hemodynamic monitoring is fundamental. Antibiotics only must be used when a respiratory infection is suspected.
