The problems due to unidentified antibodies may be due to specific red cell antigen-antibody reaction which is too weak to be identified or may be due to special problems which will be presented by Sringarm. Whenever difficulties arise in the pre-transfusion testing, they are usually serious problems. Some of them may be very simple and there is enough time to search for the causes. Unfortunately, most of the cases are urgent and are often the most confusing ones. Failure to correctly resolve these problems may well result in serious transfusion accident or delay hemotherapy. The best solution to any problem is that one should take into consideration i.e. the cause of the problem. However, human errors are more common than technical errors in a Blood Bank. Consideration must be given to all factors involved in the test system.
