Recurrent bronchopulmonary diseases in CF children lead to the antibody production. In our group of patients the IgG and IgA values were strikingly higher compared with the values in healthy children. Hyperimmunoglobulinemia is prognostically a bad sign. The mean Ig values showed statistically significant decrease (p<0.02). The IgD concentration decreased sharply already 6 months prior to death. IgD plays its role in the differentiation of human B lymphocytes which can change before death; it shows also a lability towards the effect of proteolytic enzymes. The inflammatory disease and the presence of pathogens in the respiratory tract activate PNM leucocytes and the proteinase activity in plasma increases. Al -AT, the main proteinase inhibitor was elevated 2 years before death of children, its concentration descreased to the upper limit of the healthy population one year before death, but prior to death the Al -AT concentration was increased (p=0.10). Also the TIC level in serum of CF children remained elevated throughout the period investigated, still insignificantly increasing closely to death. The A2-M serum concentration elastase inhibitor - wavered during the follow-up and decreased significantly (p<0.05) closely to death. As the most serious sign of approaching death in CF children we consider a marked decrease of IgD values in serum and decrease of A2-M values.
