The blast cells from a patient with Ph 1-positive chronic granulocytic leukemia were analyzed by a panel of monoclonal antibodies and also according to morphological and cytochemical criteria. Four sequential phenotypic changes (lymphoid, granulocytic, erythroid and granulomonocytic) were documented. The capacity of these blast cells to differentiate along several pathways indicates that CGL is a condition derived from a pluripotent hemopoietic stem cell. The importance of using a combination of morphological, cytochemical and immunological approaches for the correct diagnosis of these cases, thus facilitating the possibility of using specific chemotherapeutic drugs according to the type of blast transformation, is discussed.
