Various aspects of phagocyte participation in the pathogenesis of pulmonary abscesses and gangrene

Patients with acute (38) and chronic (8) pulmonary abscesses and pulmonary gangrene (10) were examined. The increased mononuclear count indicates a decline in the activity of the pyodestructive process, but it does not determine the long-term prognosis. The nitroblue tetrazolium test of pulmonary macrophages in acute purulent pulmonary destruction may be used for predicting the possibility of the diseases becoming chronic. The level of procoagulant activity of blood monocytes and pulmonary macrophages does not correlate with the activity of the pyodestructive process. The proteolytic activity of leukocytes washed from the purulent destruction focus in the lung is determined mostly by proteinases of polymorphonuclear leukocytes and correlates with the activity of the pyodestructive process. The high proteolytic activity of lavage supernatant indirectly indicates the insufficient effectiveness of inhibitors in the focus and directly reflects the level of leukocyte proteinase aggressiveness with respect to pulmonary stroma structural proteins.
