Report of chloroquine resistance malaria in Niger

Five cases of malaria are reported from French patients receiving chloroquine (CQ) prophylaxis (100 mg/day) in Niger where resistance had not previously been reported. Case one was a 27-year-old man who was admitted to hospital 13 days after his return to Paris from Niger. Cases 2, 3, 4 and 5 included 4 young adults in charge of a field research tour, in the area of Tillaberi, along the Niger river.They had malaria attacks 3, 7, 10 and 14 days after their direct flight back to France. In all the 5 cases, on admission, treatment was started with quinine iv (25 mg/kg/day) which was switched to oral treatment when Plasmodium falciparum had been cleared. All were cured and during follow-up, 2 months later, thick blood smears remained negative. The strains of P. falciparum isolated in patients 1, 2 and 5, were CQ resistant in vitro.
