Twa female patients developed Respiratory Distress Syndrome suddenly after the ingestion of hydrochlorothyazide. Other etiologic factors were excluded, and pharmacologic interaction probability algorhytms supported the diagnosis. In addition, a positive blastic transformation test was obtained in one patient. Evidente from these and other cases reported suggest an idiosyncratic reaction of unknown mechanism as the etiology of this adverse reaction. An immune mechanisms is likely given the development of noncardiogenic pulmonar edema, fever and bronchial obstruction within the first our after drug ingestion in presumably sensitized individuals. Recurrente of the reaction after a new drug challenge further confirms this hypothesis.
