Coccidioides immitis: an unusual presentation

A case of C. immitis osteomyelitis, arthritis and extraarticular soft tissue extension is reported in a 33-yr-old man who presented with a 4-month history of increasing left knee pain and swelling. Previous history included disseminated coccidioidal infection 7 yrs earlier with involvement of the left medial femoral condyle. Radiographs revealed a focal lytic defect in the left lateral tibia plateau and sclerotic changes in the left medial femoral condyle. Surgical debridement was performed and C. immitis was cultured from fluid from the knee and from tissue. Treatment with fluconazole (100 mg/d orally) was given and 18 months postoperatively, radiographs showed bony healing of the tibial lesion.
