Amphotericin B overdoses in infants: is there a role for exchange transfusion?

A case is reported in a 5-month-old male infant who received amphotericin B for suspected fungal meningitis following bone marrow transplantation. Although 2.5 mg of drug should have been administered, the patient received 31 mg as a single dose. Due to concerns for potential nephrotoxicity, a 2-volume exchange transfusion was performed. This did not result in a significant reduction in plasma levels of amphotericin B. However, following the overdose, renal function remained normal, no new laboratory abnormalities developed and no residual clinical effects were noted. The role of exchange transfusion in infants overdosed with amphotericin B is questioned.
