The most important point in our strategy is to prevent parathyroid hyperplasia from its early phase, thus keeping secondary hyperparathyroidism controllable by calcitriol therapy. The management should be especially focused on the prevention of the progression from typeIII to type-IV hyperparathyroidism. Thus, early use of active vitamin D [22], calcitriol pulse therapy and/or phosphorus restriction is recommended. The changes in parathyroid gland size as well as PTH hypersecretion should also be taken into account in the management of secondary hyperparathyroidism.
