Despite surgical efforts over a century, peritoneal adhesions are still an unsolved problem. Pathogenetic aspects derived from an analysis of the literature (700 papers, 1888-1989) are discussed: Two basic mechanisms play a role. Inhibition of the physiological regeneration of mesothelial defects and depression of peritoneal fibrinolytic activity mainly by ischemia and inflammation. Fibrinolysis in due time prevents the fibrinous adhesions from being transformed into fibrous ones. - All prophylactic and therapeutic measures have to be evaluated regarding these factors.
