A case of acute cardiac insufficiency in a 21-year-old immunocompetent woman is reported. The onset was sudden with thoracic oppression, severe dyspnoea and fever that started the day before her return to France from a holiday in Kenya (under malaria prophylaxis with mefloquine). Her condition was attributed to toxoplasmic myocarditis on the basis of clinical presentation, cure following symptomatic and specific therapy (spiramycin and methylprednisolone), serology and echography. Acute myocarditis associated with toxoplasmosis, as well as the presence of focal involvement of the septum are extremely rare. It is suggested that toxoplasmosis should be suspected in acute heart failure when other causes have been eliminated.
