Oligoarthritis B27 with edema and general symptoms: an atypical form of spondylarthritis with late onset

This atypical form of spondylarthritis was described after studying 11 patients. The latter are males over 50 years old, with minimal or no axial involvement, asymmetrical oligoarthritis of the lower limbs, few cells in the synovial fluid and a fibrous synovium upon biopsy. In contrast, their general condition is greatly affected, with an elevated erythrocyte sedimentation rate and unexplained edemas suggesting diverse non-rheumatological diseases. Bilateral sacroiliitis appeared in 6 patients and 5 developed ankylosing spondylarthritis.
