Acute histoplasmosis: a modern appraisal

Two acute cases of Histoplasma capsulatum pulmonary infection are reported which followed exposure to bat guano during examination of a tunnel in 1988. Bronchial washings and transbronchial biopsies were negative for the fungus. Serial CFT suggested recent infection. The infections were quickly and effectively controlled by oral daily doses of 400 mg ketoconazole.
