Urinary catecholamine excretion during one-lung ventilation and two-lung ventilation

We measured excretion of norepinephrine (NE), epinephrine (E) and dopamine (DA) per 1 mg of urine creatine in order to evaluate the sympathetic activity during one-lung ventilation in 15 patients and during two-lung ventilation in 12 patients. These patients underwent elective thoracotomy under enflurane with nitrous oxide and oxygen anesthesia. Catecholamines were analyzed by high performance liquid chromatography. The results were as follows: 1 In two-lung ventilation, there was an increase in NE excretion ion response to surgical stress, changing from 78 .+-. 34 (mean .+-. SD) ng/mg Cr to 108 .+-. 58 ng/mg Cr (P < 0.05). But we obsered no significant increase in E and DA excretion. 2 In one-lung ventilation, there was an increase in NE and E excretion in response to urgical stress, changing from 85 .+-. 37 ng/mg Cr to 160 .+-. 78 ng/mg Cr (P < 0.01) and from 18 .+-. 8 ng/mg Cr to 29 .+-. 12 ng/mg Cr (P < 0.01), respectively. Present results suggest that this one-lung anesthesia procedure increases the sympathetic activity during operation.
