Twiddler's syndrome--a pacemaker complication

A case of twiddler's syndrome is described. This is a rare complication of pacemaker treatment in which coiling of the electrode around the pacemaker occurs. In our patient, this resulted in displacement of the electrode with subsequent undersensing, exit-block and muscular contractions. The syndrome occurs particularly in cases with large pacemaker pockets and, in this particular case, it was due to abundant, loose subcutaneous adipose tissue and manipulation of the pacemaker by the patient (.mchgt.twiddling.mchlt.).Prophylactic measures including the smallest possible pacemaker pocket and possibly also fixation of the pacemaker are discussed.
