Lactic acidosis and insulin resistance associated with epinephrine in a patient with noninsulin-dependent diabetes mellitus

The case of a 22-year-old male AIDS patient with Pneumocystis carinii pneumonia from Ohio, USA, who developed severe lactic acidosis following infusion with isoproterenol is reported. The patient was found to be hypomagnesaemic and hypocalaemic before central hyperalimentation. The acidosis resolved with discontinuation of isoproterenol, and administration of bicarbonate and insulin. It is concluded that beta -adrenergic stimulation may cause lactic acidosis in patients receiving aggressive catecholamine therapy.
