Effect of the size of the necrotic mass on the pumping and contractile function of the left ventricle in patients with acute myocardial infarction

A total of 130 patients with primary acute transmural myocardial infarction were examined and separated into 3 groups according to the size of infarct. Dependence of certain parameters of pumping and contractile failure on the size of necrosis was studied. The incidence of acute cardiac failure and mortality rate largely depended on the size of infarct, while this index had no correlation with cardiac output parameters. There was functional heterogeneity of intact left-ventricular parts of the posterior wall and interventricular septum in particular. A correlation was found between the mass of necrosis and the end diastolic left-ventricular index and volume/pressure coefficient.
