In groups of 7 to 15, 272 men and women, 29 to 72 years old, with hypertension stage I to III and varying degrees of obesity were assigned to a diet with 1300 to 1800 kcal daily with 1 to 2 days relief weekly or with fasting 1 day in 7 or 10 days and counselled on diet adherence and self-control. Diets had protein 80 to 85, fats 75 to 80, unrefined carbohydrates 180 to 200 and salt 4 to 6 g daily. Patients also followed their normal drug treatment. After 3 to 12 months or more of treatment weight loss was more than 10%, 5 to 10% or less than 5% of excess weight in 16.8, 42.0 and 41.2% of patients, respectively. Mean weight loss was 30.1 to 32.3 kg/msuperscript 2. In patients with stage I and II hypertension there was a decrease in arterial pressure. In stage III hypertension the decrease was less marked and less stable. There was a direct relation between decrease in body weight after treatment and requirement for hypotensive drugs.
