Alteration of platelet function in myeloproliferative disorder and its clinical significance

Control study of platelet adhesiveness and aggregations induced by ADP, epinephrine, collagen and ristocetin in 20 patients with CMPD and 20 healthy volunteers was done. The platelets from thirteen patients and the eight volunteers were examined by transmission electron microscopy. The results showed that the platelet aggregation rates of the patients induced by ADP, epinephrine, collagen were lower than those of the control group. Platelet adhesiveness of the patients was lower than that of the control group. Basing on our study, we are unable to demonstrate a definite correlation between platelet function alteration and clinical bleeding or thrombosis. The electron microscopic examination showed that the number of dense body of the platelets in patients with CMPD was lower than those of the control, but the .alpha.-granule existed no significant difference between them. Our result confirms the existence of storage pool deficiency in CMPD, which may account for the abnormalities in platelet adhesiveness and aggregation. The platelet aggregation rate induced by ristocetin was significantly lower in the patients with CMPD, we speculate that the acquired von Willebrand's disease might be one of the factors of bleeding in the CMPD patients.
