The Guadalajara Study (3) : Changes in clinical and analytical parameters in diabetic patients exposed to a course of continuing diabetes education. Results of 30 months' follow-up

We have analyzed the changes in biochemical and clinical parameters in 111 diabetic patients treated with insulin, having followed for 30 months a program for continuous diabetological education developed at a primary care center. Initial HbA1 decreased from 11.82 .+-. 2.09% to 9.42 .+-. 2.31% (p < 0.001), 9.25 .+-. 1.47% (p < 0.001), 9.03 .+-. 1.26% (p < 0.001) and 9.44 .+-. 1.38% (p < 0.001) after 12, 18, 24 and 30 months, of follow up respectively. Thirty-three patients had a mean HbA1 lower than 8.5%. In these patients there was a reduction in body mass index (BMI) (24.13 .+-. 4.71 vs 22.34 .+-. 4.05; p < 0.001), mean blood pressure (10.60 .+-. 2.20 vs 9.31 .+-. 1.39; p < 0.001), serum cholesterol (233.06 .+-. 49.44 vs 196.61 .+-. 36.85 mg/dl; p < 0.001), serum triglycerides (131.73 .+-. 54.60 mg/dl vs 105.51 .+-. 36.55 mg/dl; p < 0.001), proteinuria (0.294 .+-. 0.870 g/l vs 0.127 .+-. 0.523 g/l p < 0.001), while serum creatinine did not show changes (0.97 .+-. 0.44 vs 0.98 .+-. 0.73 mg/dl; n.s.). The present study demonstrates the efficacy of continuing diabetological education program to achieve prevention of the development of late complications of diabetes mellitus (at least the renal ones) through an optimized metabolic control.
