Today cardiologists' awareness about proarrhythmic effects of antiarrhythmic agents is increased. Authors report a clinical case concerning a severe proarrhythmic effect of flecainide, a class 1 C agent, recently introduced into Italy, with FDA approval for ventricular tachyarrhythmias but tested also in supraventricular tachyarrhythmias. They point out that, if a severe heart disease is present, a dramatic proarrhythmic effect may develop even with the lowest recommended doses.
