Clinical effectiveness of minoxidil in patients with severe forms of arterial hypertension

Examination included 14 patients with arterial hypertension of different etiology which was resistant to treatment with convectional hypotensive agents. The symptoms of malignant hypertension were found in 7 patients and the decrease in urea clearance and concentration renal functions in 2 patients. Minoxidil in a daily dose of 5-60 mg (23.9 mg/24 h, on the average), obsidan (120 mg/24 h) and furosemide (40-80 mg/24 h) have been used for the treatment. The daily dose of minoxidil was divided in two parts to be taken with a 12-hour interval. The follow-up period lasted from 3 months to 3 years. The adequate hypotensive effect obtained in all the cases was attended by manifest positive changes in the fundus oculi of patients with malignant hypertension syndrome. The treatment improved oxygen tissue supply and capillary circulation rate. This combination of agents caused side effects: inversion of the T wave on the ECG and hypertrichosis. Pericardial effusion was not found in any of the patients.
