Occurrence of urethral stricture in patients submitting to vascular surgery. Is urethral ischemia an important pathogenetic factor of urethral stricture?

The incidence of urethral strictures, secondary to catheterization during surgery for occlusive arteriosclerosis of the lower limbs was determined in 71 patients 12 to 34 months postoperatively. Twelve patients suffered from infravesical obstruction with flow < 10 ml/s. Three patients had urethral strictures confirmed by flow investigation and cystoscopy or urethral calibration (4%, 95% confidence limits 1-12%). Forty-three of the patients complained of erectile impotence at the time of surgery and it is assumed that many of these patients had a low penile blood pressure. Although recent investigations have suggested that low penile blood pressure and ischemia of the urethra are predisposing factors in the development of post-operative catheter-induced urethral strictures, the present investigation results do not support this assumption.
