A case of intestinal myiasis in a 26-year-old man, attributed to larvae of Musca domestica, is reported from India. The patient presented with a year's history of passage of 10-50 "worms" daily in his stool, abdominal pain (mainly in the epigastric and umbilical regions), increased appetite, weakness, pain in his calf muscles and lack of concentration. Following provisional diagnosis of helminth infection but negative evidence for this, the patient provided maggots which he stated had come from his faeces; subsequently, maggots were found in freshly deposited faeces (within 2 min of defaecation). The patient was given a dose of 60 ml castor oil, followed by a second dose 10 days later, but he continued to pass maggots.
