Electrophysiologic studies of ventricular tachycardia in cardiomyopathy

The mechanism of ventricular tachycardia (VT) in cardiomyopathy are unclear. This report is to dicuss our results in 11 patients with VT, investigated by endocardial mapping and programmed stimulation. The results showed that : (1) among the 8 cases studied by endocardial mapping during sinus rhythm, VT or cardiac pacing, the ventricular late potentials (VLPs) were demonstrated in 6 cases of septum or apex; (2) six patients with recurrent VT were terminated by programmed and rapid ventricular stimulation; (s) marked acceleration of VT occurred in one case during attempted termination by brief bursts of rapid pacing. These data indicate that most of the cases with recurrent VT result from intraventricular reentry only one may be caused by ectopic focus of increased automaticity. The VLPs reflect local intramuscular conduction defect. Therefore, marked intraventricular conduction abnormality and VLPs indicate a susceptibility to reentrance with no special respect of the type of cardiomyopathy. 3 cases of familial arrhythmogenic right ventricular cardiomyopathy have been reported, it is a hereditary disease characterized by isolated right ventricular dilatation, recurrent VT and delayed potentials demonstrated on right precordial leads and right ventricular endocardial mapping.
