Intracranial hemorrhage in the neonate--a clinical and ultrasonographic study

Eighty six neonates with various gestational ages were screened for periventricular intraventricular hemorrhage (PVH/IVH) with ultrasonography. Thirty five normal neonates served as controls, two of these had a silent Grade I hemorrhage. Out of 51 neonates at high risk for PVH/IVH. 20 showed hemorrhage: 6 had Grade I, 4 had Grade II, 7 had Grade III and 3 had Grade IV hemorrhage. Massive hemorrhages could be clinically predicted by the catastrophic syndrome and were seen at lower gestational ages. A minimum of 3 scans around 4 days, 14 days and 3 months were essential to judge the evolution of the hemorrhage. Neurodevelopmental follow-up showed that neonates with Grade I hemorrhage were normal, whereas one survivor in Grade III and one in Grade IV were handicapped.
