Intracranial hemorrhage in term neonates

Thirteen term neonates with intracranial hemorrhage (ICH) were studied. Of these 5 had subdural, 3 subarachnoid, 3 intraparenchymal, 1 intraventricular and 1 subependymal hemorrhage. Three infants expired, 6 were normal and 4 were handicapped at 1 year. Hypoxic-ischemic encephalopathy (HIE) was the precipitating cause in 11, severe thrombocytopenia in 1 and late hemorrhagic disease in 1. Of 11 cases with HIE, 4 had Grade I HIE all of whom were normal on follow-up, whereas of 7 with Grade II/III HIE, 2 were normal, 2 expired and 3 were handicapped. The diagnosis of ICH was confirmed on a CT scan in 12 neonates and by an ultrasonogram in 1. Of the 4 cases with moderate to severe hypodensities on CT scan, 3 were handicapped. The outcome of our patients correlated with (i) site of hemorrhage, those with subdural and subarachnoid hemorrhage had a better prognosis than those with intraventricular or intraparenchymal hemorrhage; (ii) clinical grade of HIE, and (iii) associated hypodensities on CT scan.
