Coronary thrombosis occurs in most patients with acute transmural myocardial infarction. Immediate thrombolytic therapy with streptokinase may reestablish the coronary flow, improve left ventricular function and survival after acute event. Intravenous infusion of streptokinase is easier, faster, cheaper and more widely applicable than intracoronary administration. Several studies have shown that intravenous streptokinase is generally safe, well-tolerated and moderately effective. A multicenter trial demonstrated that high-dose intravenous streptokinase achieved thrombolysis in about 40 per cent of patients with acute myocardial infarction. Rentrop reviewed that data and reported lysis rate of 45 and 84 percent in myocardial infarction patients with or without preintervention angiogram respectively. In this study, we investigated the efficacy and safety of high-dose, short-term intravenous streptokinase in acute myocardial infarction in Thai population.
