Intracoronary thrombolysis using streptokinase in acute myocardial infarction. Pilot study of 12 cases

Patients (12) with myocardial infarction of < 5 h were subjected to intracoronary thrombolysis (ICT) with streptokinase (SK). Early surgery was applied to 9 patients in order to recanalize the blood vessels. Recanalization was achieved in the 12 patients after ICT, in 1 case with nitroglycerin alone and in the remainders with SK. The mean SK dose was 171,800 .+-. 14,900 U, and the infusion time was 51.5 .+-. 1.1 min. Nine of the patients had systemic fibrinolysis, fibrinogen being reduced within 1 h to 0.32 .+-. 0.15 g/l; plasminogen was 28 .+-. 7%, fast antiplasmin was 16.5 .+-. 7%, and .alpha.2-macroglobulin was 40 .+-. 6%. FDP alterations improved immediately. Hemorrhagic complications were negligible, and re-thrombosis appeared in only 1 case during the immediate postoperative period. The remaining 8 patients subjected to surgery and coumarin anticoagulation therapy had permeable bridges in the angiographic controls performed 1 yr later.
