A case of right traumatic diaphragmatic hernia

A 38-year-old man was injured because of falling from the height of 5 meters. He was diagnosed as right pneumo-hemothorax, right rib fractures, (7th-9th), right traumatic diaphragmatic hernia, hematoma of the liver, injury of the kidney and compression fracture of the third lumbar vertebral body. There were no intraperitoneal organ injury and severe respiratory disturbance that required emergent operation, and he received conservative therapy with continuous drainage of right chest. Firstly he received the operation for compression fracture of the third lumbar vertebral body on twenty days after the accident to prevent the spinal injury, and so he was treated for right diaphragmatic hernia on seventy days after the accident. Operation with thoracotomy revealed the protruding right lobe of the liver into the right thorax. Liver was returned into the intraperitoneal cavity, and the defect of the diaphragm was repaired with Teflon felt patch. Postoperative course was uneventful, and the expansion of right lung was sufficient.
