Plasmodium malariae parasites were found in the blood of a 70-year-old man with fever who was born in Israel and had never been outside the country. The malaria was presumably acquired by transfused blood from an asymptomatic donor in whom antibodies to malarial parasites were detected by radioimmunoassay. Transfusion malaria should be considered in the differential diagnosis of febrile illness appearing after the transfusion of blood or blood components.
