Acute renal insufficiency during an episode of macroscopic hematuria in IgA glomerulonephritis

Acute renal failure associated to episodes of macroscopic hematuria in IgA glomerulonephritis (IgAGN) has been usually considered uncommon. However, in recent studies it has been pointed out that the frequence of this complication is remarkable (38%). The histolgical changes best correlated with this type of impairment of renal function are the presence of blood casts in the tubular lumen and findings of tubular necrosis. It has been suggested that mechanical obstruction by these casts and/or their toxicity on tubular cells could play a significant pathogenetic role. In the case reported in this article, that confirms the previously reported data, there were remarkable features such as the severity of renal failure (plasma creatinine 5,1 mg/dl), the long duration of macroscopic hematuria (35 days), and the absence of recovery of the previous level of renal function.
