Left ventricular function and intracardiac blood flow in acute myocardial infarction

General and segmental functions of the left ventricle and indices of the intracardiac blood flow were studied in the dynamics of a disease and their role in the development of cardiac insufficiency was determined on the basis of a comprehensive study of 76 patients with acute large focal myocardial infarction of the anterior localization by methods of sector scanning and two-dimensional pulsed doppler-echocardiography. The results suggest that disorders of the segmental function of the left ventricle in the acute period of myocardial infarction produce changes in the velocity of the intracardiac blood flow in the left ventricular cavity that depends on the size of the lesions, the degree of depression of the affected myocardium contractility and the condition of the intact regions of the left ventricular wall. It was found that a sharp slowing of the blood flow velocity in the efferent tract of the left ventricle indicates a pronounced depression of both the general and segmental contractility of the myocardium and correlates well with the development of cardiac insufficiency. A marked decrease of the velocity of the blood flow in the region of the heart apex in the first 24 hours of the disease and the absence of augmentation of this index in the dynamics of the disease is a valid sign of the formation of acute cardiac aneurysm.
