A study of bile enterotoxin in cholelithiasis

The purpose of this study was to determine the titer of intra-operative and post-operative bile enterotoxin in cholelithiasis by a reversed passive latex agglutination test. According to titer results, can be determined and evaluated prognosis and effects of treatment in cholelithiasis. The intra-operative bile of 45 cases (18 male, 27 female) of cholelithiasis were tested. The positive rate of bile enterotoxin was 71.4% in 14 gallbladder stones, 100% in 15 common bile duct stones and 16 in intrahepatic duct stones. The correlation between bile enterotoxin and bacterial culture were evaluated as well with forty one cases of positive bile enterotoxin having 70.3% of positive bacterial culture, but 4 cases of negative bile enterotoxin having only 25% of positive culture. Thus, a close correlation was indicated. The enterotoxin titers in post-operative bile was apparently decreased in 4 cases of 5 common bile duct stones one week after surgery (p < 0.05) and in 7 cases of 10 with intrahepatic duct stones the same results occurred. One case had an temporary post-operative titer elevation, but soon became negative after the removal of a residual stone through choledochoscope one month later. This study, therefore indicated that titer levels can be used for the evaluation of the effects of management and prognosis in cholelithiasis.
