Paecilomyces lilacinus keratitis: two case reports in extended wear contact lens wearers

Two cases of Paecilomyces lilacinus keratitis associated with aphakic extended wear contact lens use are presented. Successful visual outcome of 20/40 in both cases is attributed to prompt and prolonged medical treatment, followed by delayed penetrating keratoplasty, use of imidazole-type antifungal agents, and limitation of corticosteroid administration. A review of other reported cases suggests this therapeutic approach may improve the usually poor prognosis associated with corneal infection by this organism.
