Anesthetic management of patients with DIC

This report describes seven patients who developed "disseminated intravascular coagulation syndrome (DIC)" recently. All patients were with advanced cancer and multiple metastases. They had microangiopathic hemolytic anemia (MHA) and died, not only with disorders in blood coagulation and the fibrinolytic system but also with remarkable elevation in serum alkaline phosphatase and lactate dehydrogenase, leukoerythrocytosis, anisocytosis and poikilocytosis of the red cells seen in peripheral blood films. We must assess the preoperative status of the patients who have recurrence and or metastasis of cancer, considering them to be in preparatory state of DIC, suspecting existence of MHA, and taking operative risk, prognosis and quality of life into consideration.
