Histological study of corneal buttons removed because of pseudophakic keratopathy regularly reveals a dramatic loss of endothelial cells. In addition, different combinations and stages of the following reactive processes are observed in the endothelial layer: attraction of polymorphonuclear leukocytes and macrophages; settling of fibroblast-like macrophages; transformation of settled macrophages into epithelioid and giant cells; deposition of a proteinaceous film; bleeding; and proliferation of sessile macrophages, with formation of fibrous retrocorneal membranes in the late stages. These changes in the endothelial layer are clearly related to the reactions on the surface of lens implants that have been described in earlier reports.
