Genital actinomycosis in women wearing an IUD

Clinico-pathological examinations were carried out to throw light on the connection between intrauterine contraceptive and genital actinomycosis (AM) a rather frequent inflammatory complication today. During 12 years more than 20,000 IUD-s were placed in and AM was suspected in 20 cases on the basis of the histological examinations performed from the operative- and curettage material. In fourteen (10.5%) of 133 patients (0.65%) subjected to surgical treatment AM was suspected. Applying selective tissue staining AM could not be confirmed in half of the "blocks" thought to be AM (pseudoactinomycosis-PAM). Cases which were suspected of AM could not be distinguished clinically from the other surgical cases. Distributed between true AM and PAM actinomycosis was suspected in approximately 7% of the tissular scraping materials obtained from the surface of 156 devices eliminated because of different causes. In the material obtained from vaginal cytological check examinations (n = 1000) AM-like structures could be revealed in low number in women wearing and not wearing devices (1.6 and 1.0%) resp., Clinico-pathological observations and examinations in accordance with other modern microbiological and immunological studies suggest that there are no convincing causes which would attribute a particular role to the rather frequent genital actinomycosis in the inflammatory diseases of the true pelvis occurring as a complication of the use of IUD.
