Persistent atrial fibrillation during insertion of central venous catheter with the catheter-over-guidewire device

The catheter-over-guidewire device has been widely used for the central venous catheterization. This technique has the advantage over the catheter-through-cannula technique; the initial puncture of the vein can be performed with a smaller gauge needle. However, the use of guidewire is unavoidable for the placement of the catheter. We describe a patient in whom persistent atrial fibrillation occurred immediately after the insertion of guidewire via right subclavian vein. This arrhythmia was successfully reverted to normal sinus rhythm with DC counter shock. The possibility of causing arrhythmia by the use of guidewire is emphasized in the present paper.
