This study is considering the thromboembolic prophylaxis of 31 patients with prosthetic heart valve. The patients were treated with coumarol plus small dose aspirin (2 .times. 500 mg ASA/week). Prothrombin ratio was used for control of the oral anticoagulant therapy. Malondialdehyde (MDA) production and the so-called MDA-ratio (patient's plasma MDA level/control plasma MDA-level) were measured, parallelly. Platelet aggregation study and examination of thromboxane and prostacycline metabolites were also carried out. No case of gastrointestinal bleeding or thromboembolism was observed during the 30 months follow-up. Combined (anticoagulant plus antiplatelet) therapy is suggested in patients with prosthetic heart valve. For laboratory control examination of the prothrombin and the malondialdehyde ratio together with platelet aggregation is recommended.
