A correlative study of thallium-201 myocardial imaging, electrocardiography and coronary arteriography in 50 patients with coronary heart disease

Thallium-201 (T1-201) myocardial imaging was performed in 50 patients with coronary heart disease and the results were compared with electrocardiographic (ECG) and coronary arteriographic findings. T1-201 imaging at rest was obtained in all patients and 21 of them during submaximal exercise testing. There were 44 cases with coronary artery stenosis (more than 50% narrowing) and 6 cases with normal coronary artery confirmed by arteriograpy. Overall, the coincidence rate of T1-201 image and ECG at rest to coronary arteriographic findings was 94 and 80 percent of cases (P < 0.05) respectively. T1-201 image and ECG at rest and similar sensitivity in detecting myocardial infarction, however, the specificity of rest T1-201 image seems to be better than that of ECG. Of 17 cases with positive responses by exercise ECG, 2 cases who had normal coronary artery and had no defects by exercise T1-201 image. Of another 4 cases with normal coronary artery, 2 cases who showed positive responses by stress ECG did not find defects by stress T1-201 image. According to this study, the sensitivity and specificity of rest or exercise T1-201 myocardial imaging are higher than the rest or exercise ECG in detecting myocardial ischemia.
