Glucose, lactic acid and pyruvic acid levels of the blood and cerebrospinal fluid in patients with diabetes mellitus and cerebral infarction

In 19 healthy subjects, 20 patients with cerebral infarction without diabetes, and 29 patients with cerebral infarction and diabetes the concentrations of glucose, lactic acid and pyruvic acid were determined in blood and cerebrospinal fluid. The present investigations confirmed the observations of high glucose levels in blood and cerebrospinal fluid, in fasting state, and increased levels of lactic and pyruvic acids in cases of cerebral infarction not treated for diabetes, with the increase of lactic acid level greater than that pyruvic acid in blood and cerebrospinal fluid. It was shown that in patients with cerebral infarction treated for diabetes a significant rise occurs in the glucose levels in blood and cerebrospinal fluid. The blood level of lactic acid is increased. In the cerebrospinal fluid the level of lactic acid in these patients is similar to that demonstrated in non-diabetic patients with cerebral infarction. On the other hand, the concentration of pyruvic acid in the cerebrospinal fluid in diabetics with cerebral infarction was significantly lower than in healthy subjects and non-diabetic patients with cerebral infarction. The obtained results may suggest greater disturbances of carbohydrate metabolism of anaerobic type in diabetics with cerebral infarction development.
