First results of ileoanal anastomoses with the construction of an ileal reservoir are presented. In 24 patients the operation was attempted:8 with colitis and 16 with familial polyposis coli. There was one postoperative death. One patient with polyposis coli and a preceding colectomy with ileorectal anastomosis was inoperable owing to huge intra-abdominal desmoid tumours. Postoperative complications included pelvic abscess in one patient and postoperative bowel obstruction in three patients. Fifteen patients have had their ileostomy closed. They are all continent during the day; four have to wear a pad at night. Frequency of defaecation ranged from 2-10 per 24 h. No patient has perineal excoriation.
