A case of congenital leukaemia is reported, including ultrastructural, cytochemical, immunologic and cytogenetic findings. The cell morphology was that of lymphoblastic leukaemia, although it was cytochemically undifferentiated. The study of surface markers was not conclusive, but it was suggestive to correspond to common, non-T non-B lymphoblastic leukaemia. A higher incidence of lymphoblastic leukaemia in the perinatal period than previously admitted is suggested after those transient cases associated to Down's syndrome are excluded. On occasion, the proliferating cell clone in congenital leukaemia may involve the stem cell or differentiating cells close to it.
