Equatorial scleral collagen fibers of 4 excessively high myopic eyes and 5 nonmyopic eyes were examined by electron microscopy. In architecture studies, the fiber bundles seen in myopic sclera were frequently of thin lamellar arrangement, whereas those of nonmyopic sclera were of interwoven or thick lamellar arrangement. In fibril studies, the collagen fibrils in myopic sclera were distributed in loose matrix with reduction in fibril diameters, as compared with the more compact nonmyopic fibrils. The similar changes in ultrastructure of both equatorial and posterior staphylomatous sclera suggest a generalized abnormal scleral fibrillogenesis in pathological myopia.
