Status of the coronary arteries and left ventricle in young patients with a history of myocardial infarction

Specific features of the coronary bed and the left-ventricular myocardium were studied in 27 patients aged under 40 with a history of acute myocardial infarction. On the basis of angiographic picture two groups were classified: 1. without stenosing changes (14) and 11, with coronary atherosclerosis (13 patients). In group I changes were found only in the anterior interventricular branch (AIVB), were atypical for atherosclerotic affection, and depended on the time when myocardial infarction had occurred. The pumping function of the left ventricle was nearly the same in both groups, through all patients of group I and only 4 patients of group II had aneurysms of the left ventricle. A causal connection of the more "malignant" myocardial affection in patients of group I is discussed as well as the development of acute myocardial infarction with a sudden obstruction of AIVB with a recovery of the arterial lumen in the future.
