Refractory anemias, preleukemia and myelodysplasias are interlinked clinical and hematological conditions. The concept of an evolutive pattern is presented indicating the major grounds basis of the transition from a condition to another reaching the full disorder, the myleodysplasia. The abnormalities seen in the erythropoietic, granulocytopoietic and megaryocytic series are described. The defective differentiation of the stem cell is considered. The means available at the moment to stimulate the differentiation of the stem cell are indicated.
