Emergency operations of abdominal aortic lesions in dissecting aneurysms of the thoracic aorta

Of 56 patients who were operated on for dissecting aneurysm of the thoracic aorta up to Sept. 1983, 4 underwent the emergency aortoiliac reconstruction for rupture or acute occlusion of aortoiliac arterial region, followed by elective operation for dissecting aneurysms of the thoracic aorta. Of the patients with dissectng aneurysm of thoracic aorta (DeBakey II), 1 had an abdominal straight graft for expanding aneurysm and then had Bentall's procedure for annuloarotic ectasia at the 2nd stage. The other 3 patients with dissecting aneurysm of the thoracic aorta (DeBakey IIIb) had an emergency aortoiliac reconstruction for rupture or acute occlusion, and a couple months following the operation, had the thoracic resectional and grafting procedures. All patients survived the operation and lead a normal life. The surgical technique of aortoiliac bifurcation grafting with proximal fenestration of the abdominal aorta just below the renal arteries, and implication of 2nd stage surgery for dissecting aneurysm of the thoracic aorta are discussed.
